If you would like to refer a patient to Dr. Parrish, please fax pertinent chart notes, labs, and imaging to 720-707-0210, including the patient's full name, date of birth, phone number, and reason for referral. Alternatively, you may send inquiries to info.boulder@nawellness.com. We will contact the patient to schedule an appointment.
